The owner assumes full responsibility for the dog’s behavior during and after the training/counseling program. *RESPONSIBLE PET OWNER'S PLEDGE (DISCLAIMER): The owner understands and agrees that they are being trained to train their dog(s) and without 100% cooperation with the trainer(s); the dog(s) will not get trained. I have read and understand that all training is contingent upon the owner's willingness to handle and/or work with their dog(s) as described above. *PET TAXI LIABILITY: You may verbally (by telephone) or in writing (by email or otherwise) request that SMP Paws release your pet to someone other than the person(s) listed, and you release SMP Paws of and from any and all responsibility for releasing your dog to any person to be authorized by yourself. During Pop-up Daycare (Bootcamp), Santa Monica Paws will release your pet to the following person(s) listed: *AGGRESSIVE BEHAVIOR DISCLAIMER (if applicable): I acknowledge the Company has determined that my dog(s) may be aggressive or is aggressive. The Company has fully explained that since the outcome of any training/counseling program cannot be guaranteed and the aggressive behavior on the part of my dog(s) is or may become a risk to people and/or dogs, my decision to enroll in a training program is done solely and completely at my sole discretion with me taking 100% responsibility for any damage my dog(s) might cause to anybody during or after the training program. The Company does not promise to solve or curtail my dog’s aggressive behavior. I further agree to take all possible safety measures recommended by my trainer, including but not limited to muzzling and leashing my dog(s) when around dogs and/or other people. *REFUND POLICY Termination of Training: the owner of the dog(s) may have the animal withdrawn from training at any time. Once payment has been received, you have 48 hours to cancel to receive a full refund. prior to training. Please note, all fees must be paid prior to training, and a binding Training Agreement has been signed; before training can begin. All fees are non-refundable. It is the discretion of Santa Monica Paws to grant refunds if they find suitable once 48 hour grace period has expired.***Please note a $150 refundable incident fee to cover any accidents is expected via credit card( please enter credit card #, expiration date & 3 or 4 digit card code below), Zelle, Cash. This covers your pup should they find themselves in a circumstance that requires grooming. If no accidents, the fee is fully-refunded. *EMERGENCY MEDICAL CARE: In our judgment, if your dog requires medical care and we are unable to reach you, please indicate below whether you want us to take your dog to a veterinarian or animal hospital of our choice. LIST ALL SPECIFICATIONS HERE. * *MEDICAL TREATMENT: The owner/agent authorizes emergency medical care to be provided by their own vet. In the event their veterinarian is unavailable the owner/agent authorizes the Company to have the dog(s) given emergency medical treatment by any licensed veterinarian of the Company’s choosing. The client will reimburse the Company for all charges related to this emergency care provided receipts are submitted to the owner. Upon receipt of such receipts, the owner/agent will pay all amounts noted on receipt(s) to the Company within fifteen days. *RELEASE OF LIABILITY: I agree to be solely responsible for the payment of all medical bills for my pet and I agree to release Santa Monica Paws, its officers, directors, agents and employees of and from any and all responsibility for, or claims, damages, debts, arising out of or related to such medical care, including, but not limited to, transportation to/from the veterinarian clinic and choice of veterinarian or animal hospital. I agree NOT to hold Santa Monica Paws liable *(if I write a bad review on any social media platform defaming Santa Monica Paws for unsupervised actions) if I misuse any recommended tools for training that result in injury to my pup. I agree to release Santa Monica Paws, of and from any and all responsibility for or claims, damages, debts; arising out of or related to Santa Monica Paws NOT providing or obtaining medical care for my pet and I acknowledge that Santa Monica Paws is not required to give any medical aid without my consent. This includes any injury, death, sickness or damage my pet may suffer during or after any training program. I also agree to indemnify and hold harmless the Company and Theresa Chern & Brad Dean from any and all claims due to damage the pet may cause to any family members of any third parties during or after training. If for any reason there become large gaps in pup attendance, a re-evaluation to determine temperament and sociability is required due to the disruption that comes with pups who miss critical socialization development when reintroduced to a socialized pack. *BORDETELLA (Kennel Cough) DISCLAIMER: Although SMP Paws requires my pet to receive Bordetella (kennel cough) on a yearly basis, some vets recommend that Bordetella be given on a six-month basis. Even after receiving Bordetella, your pet may develop some strains of the virus. The vaccine does not cover all strains of the virus or guarantee that your pet will not acquire the virus. Even though, SMP Paws is a mobile business, it does have public areas in which more than one animal may use at a time, which include but are not limited to communal dog fountain, dog parks, city streets, swimming pools, beaches, public sidewalks, and public play time areas. SMP Paws assumes no financial responsibility and will not be held financially liable for any Veterinary Expenses incurred on your pet 's behalf while your pet is in the care of SMP Paws or immediately following your pet's training sessions with SMP Paws, this includes but is not limited to kennel cough, viruses, fleas, or accidents incurred while a resident at SMP Paws. *CANCELLATION POLICY: If you have paid for your service ahead of time, and you cancel prior to service being rendered, partial refunds (50%) are given if the cancellation is made at least 4 days prior to the start of a reservation. This process takes net 30 days to process. We reserve the right to end the training evaluation if it is determined that we cannot help you (un-refundable). No refunds will be issued for cancellations made within the 3 day (72 hours) window, including same day cancellations. No refunds will be issued for same day cancellations. A credit will be given towards future services. Pop-Up Daycare/Bootcamp Enrichment Packages, Grooming, and all Private Training sessions can be terminated at any time, however, they are final sale and no refunds will be given once payment has been made, and at least one session has been granted. We reserve the right to cancel a reservation and to refuse any customer we deem unsuitable or that puts ourselves or our clients in jeopardy. 24 HOUR POLICY: In an effort to improve our dog training services, cat sitting, pet daycare & pet boarding and in an attempt to ensure timely care of all clients, the following cancellation policy will be implemented as of July 16th, 2017. All appointments made will be confirmed 1 day in advance to improve scheduling and quality of service. If you wish to cancel or re-schedule your appointment, please call us at least 24 -hours prior to your scheduled appointment. A same day cancellation fee of $100.00 will be applied to your ongoing bill if you fail to cancel or show up for any service appointment. Please attempt to show up on the day of your appointment in a timely manner, so we can provide improved service for you and other patrons. **Daycare days cannot be carried over or credited if not used within the 30-90 day requirement from receipt of payment. A re-evaluation will need to take place to determine liability and temperament of pup to be considered for re-entry into program-NO EXCEPTIONS. **Packages that lapse due to unforeseen circumstances are set to expire no more than 4 months after your first day is redeemed.
